It is the most common form of scarring hair loss seen in black women. Hair care practices, such as the use of the hot comb, relaxers, tight extensions and weaves, have been implicated for decades, but studies have not shown a consistent link. Cicatricial alopecia, also known as, scarring alopecia, represents various types of hair loss disorders that cause permanent hair loss. Scarring alopecia causes destruction of hair follicles and replaces the follicles with scar tissue. Once hair follicles are destroyed, hair growth is not possible. Mid‐frontal density increased improvement of associated female pattern hair loss, but there was increasing anterior hairline recession as a result of frontal fibrosing alopecia progression. After 12 months, daily ciclosporin 25 mg was initiated, and hydroxychloroquine was ceased. Dutasteride, minoxidil and spironolactone were continued. Biopsies from the pale areas devoid of hair show only fibrous tracts and not the primary pathology. The finding of gross interfollicular scarring, panfollicular lymphocytic inflammation, or involvement of the interfollicular epidermis in a biopsy should prompt a search for an alternate form of scarring alopecia that may mimic FFA.

As the exact cause is not known, should be treated with appropriate medicated shampoos and topical anti-inflammatory agents as needed. In areas where the hair follicle has been replaced with fibrosis, regrowth is not possible.
